Claude Code Max plan billing bug and Opus 4.5 elevated errors

Two new incidents: (1) Claude Code Max plan users incorrectly prompted to use usage credits for Fable 5 (monitoring). (2) Elevated error rates for Opus 4.5 (investigating). A resolved incident (elevated errors on multiple models) and a resolved partial outage were removed from the status history.
